Purpose

The Impact Tracking Template is designed to help DAO facilitators and the Kambria team monitor tangible outcomes achieved by each DAO. It complements proposal reports and summary reflections by providing clear data on what was actually delivered — trees planted, meals served, sessions held, and more.

How It’s Used

Filled out by DAO Facilitators or stewards, progressively as actions are completed
Tracks quantities, locations, dates, and documentation for each impact action
Helps produce a final cross-DAO impact summary for the end-of-program showcase and reporting

Template Structure

Column Name	Description
Proposal Title	Name of the approved proposal (should match voting title format on XDAO)
Type of Impact	Short description of the impact (e.g. "Trees Planted", "Workshops Held")
Location	Place where the impact occurred (e.g. district, online platform, community name)
Quantity Achieved	The numeric result (e.g. 25 trees, 60 attendees, 4 sessions, etc.)
Date Completed	Date the action was completed (use proposal report date or verified date)
Proposal Report Link	Link to the submitted proposal report (exported from Google Form submissions)
Verified By (Facilitator)	Name of DAO Facilitator who verified the impact
Notes	Additional comments, context, or adjustments (optional)

Managed By

The Tracking Assistant will:

Ensure consistency across DAO sheets
Populate Cross-DAO totals
Follow up with facilitators if entries are missing or unclear

DAO Facilitators can fill in their DAO’s sheet directly or coordinate with the Assistant.
