DAO Summary Report Templates

Estimated reading: 5 minutes 66 views

At the end of DEP Cohort 3, each DAO submits a DAO Summary Report documenting what it did, what changed, and what it learned. The report:

  1. documents and reflects on the DAO's work,
  2. shares impact with partners, sponsors, and the wider community, and
  3. prepares continuity for future cohorts.

The report is scored on the Proposal QA tab of the DAO Workbook (a qualitative score out of 30 and a quantitative score out of 60 → a performance tier and a decision). Most of what it needs is already in the Workbook — this page is the structure that pulls it together.

1. DAO Summary Report — Full Template

Compiled by the DAO Lead, with input from every lane.

A. Overview & DAO Profile

  1. DAO name
  2. Domain / theme
  3. Mission (1–2 sentences)
  4. Summary of the Master Plan
  5. Members and roles — DAO Lead; lane owners (by pair); DAO Facilitators (from Kambria); floating contributors; returning members
  6. Cohort duration — see the Program Timeline

B. Master Plan Overview

A short, high-level summary (max 1 page) from the approved Master Plan:

  • The DAO Objective on the three impact axes (reach · depth · self-propagation)
  • The scopes and lanes formed
  • The key milestones originally planned

C. Activities & Deliverables

C1. Activities completed — for each lane and any cross-lane activity: title, date(s), description, who was involved, partner involvement (if any), and links to outputs.

C2. Deliverables completed — the concrete outputs (content, sessions, events, materials, products, documentation, prototypes, stories). These are the deliverables marked Verified in the Workbook's Contributions tab; link to the evidence.

D. Impact Measurement (required)

D1. Objective vs achievements. From the DAO's OKR:

Impact axis Target Achieved Evidence
Reach
Depth
Self-propagation

D2. Impact stories (qualitative). Short stories showing impact on members, participants, partners, and communities.

D3. Field notes. What the numbers don't capture — the moments, surprises, and context behind the metrics. Impact isn't only what was counted; field notes are where the rest of the story lives.

E. Funds & Budget Summary (required)

A short account of how the DAO used its $3,000 seed, drawn from the Workbook’s Cashbook and Budget Plan:

  • Contributor payouts — total paid, per task, through the framework
  • Operating costs — what was spent, on what, against the plan
  • Any revenue or in-kind support brought in (from partners or activities)
  • Plan vs actual, and any unspent funds and how they were handled

F. Lane Summaries

Each pair submits a short lane summary (template in Section 2); the DAO Lead compiles them.

G. Collaboration & Partners

All organizations and communities the DAO worked with:

Partner name Type (reach / support / knowledge / community) Contribution (incl. in-kind) Evidence / link

H. KAT Recognition Summary (DAO-level)

A summary of how recognition ran — not individual allocations (those settle in the Workbook's KAT tabs):

  • Which contribution categories were used (from Workbook’s KAT Credit Rules)
  • How contributions were evaluated
  • Anything worth noting about how recognition worked this cohort

I. Challenges & Reflections

  • What challenges did the DAO face?
  • What helped overcome them?
  • What can be improved for the next cohort?
  • What new opportunities emerged?

Reflection should be honest, constructive, and future-oriented.

J. Recommendations for Future Cohorts (required)

At least five clear, actionable recommendations — covering, for example, DAO structure, ways of working, partner engagement, impact strategy, the recognition model, and member onboarding or alignment.

K. Final Statement

A short closing message: what the DAO achieved, what it hopes future cohorts build on, and acknowledgements.

L. Reflection Zine Materials (for the Showcase)

Every DEP cohort produces a Reflection Zine. The DAO Lead consolidates each lane's reflection materials into one DAO folder and confirms the five material types are complete across the whole DAO. The folder goes to the Kambria Content team, who create the DEP 3 Reflection Zine and publish it on the Update & Showcase page.

Confirm the consolidated folder covers all five types:

Material type DAO total (min) Qualification criteria
Written reflections All lane sets Every lane's written reflections gathered into the DAO folder. Each includes demonstrative photos and covers: personal thoughts & emotions; memorable moments during execution; challenges faced & how they were handled; key learnings or mindset shifts.
Video "DAO journey" ≥1 across the DAO 3–5 minutes. Shows the DAO's activities & execution moments (lane footage collected); voices of members or beneficiaries; what worked, what didn't, and what was learned.
Infographic "DAO impact" ≥1 across the DAO Covers: what the DAO did; who was impacted; key numbers / outcomes; key lessons learned. May aggregate lane infographics.
Photo album (with captions) ≥1 DAO album, ≥20 photos Different stages across the DAO: preparation, execution, people, moments. Each photo has 1–2 caption sentences giving context.
Member rating / active-member list 1 DAO table Consolidated across all lanes. Table: Member name · Contribution · Rating (1★–5★). Contribution judged on meeting participation, collaboration spirit, and responsibility.

Reference: the DEP 1 Reflection Zine sets the quality bar for what good materials look like.

2. Lane Summary Template

Submitted by each pair.

  1. Lane profile — lane name; the pair; scope (from the Master Plan).
  2. Activities completed — for each: title, description, date, who was involved, partner contributions, output(s).
  3. Deliverables completed — the lane's verified deliverables, with links.
  4. Contribution to the Objective — how the lane's Key Results moved the DAO's three axes, with evidence.
Key Result Target Achieved Evidence
  1. Challenges & learnings — challenges faced, adaptations made, what worked well.
  2. Recommendations for the next cohort.
  3. Reflection Zine contribution (this lane). Each lane produces the reflection materials arising from its own activities — the five types in Reflection Zine Materials (§L), meeting the same criteria — and submits them into the DAO's Zine folder. This is the raw content the DAO Lead consolidates for the DEP 3 Reflection Zine.
